Enable the reusable approval workflow + OTP re-authentication

The approval engine powers multi-step workflows for Documents (and later Assets, Tickets, Acknowledgements). Transitions can require OTP verification via email (always on) and optionally SMS via Twilio. Leaving Twilio blank limits OTPs to email only — safe to configure later from the admin menu.

When off, documents continue to use the legacy direct-status flow. You can flip this anytime from Admin → Integrations.
